These forms are offered in Microsoft Word 97/2000 and Adobe Portable Document Format (PDF). If you do not have Adobe Acrobat or Microsoft Word on your computer, you may view and print forms in either format by downloading and installing the appropriate viewer below.

Many of our Word forms are online forms and may be filled out on your computer using Microsoft Word, which must be installed on your computer. These online forms are denoted with a dark blue border around the icon. Do not attempt to email from within your browser using the Send command (any editing will be lost). Please save the form to your hard drive and open it in Word for editing and emailing.

To save a form:

1. Right click on the form's icon
2. Choose Save Target As...
3. Save the file in your My Documents folder
4. Go to that folder and open the file for editing
5. After editing click Save again
6. To email while in Word, click on File, Send To and choose Mail Recipient (Word 97) or Mail Recipient (as Attachment)... (Word 2000 or later).

The Adobe PDF documents offered here are not "online" forms as such. They may still be filled out using your computer if you have the full version of Adobe Acrobat installed. You may add text by using the Free Text Tool. Be sure to use the same proceedure as outlined above for saving and editing your document.
